ForeseeHome is cleared for patients who have a best corrected visual acuity of 20/60 or better and an intermediate dry AMD ICD-10 code (H35.3112, H35.3122, or H35.3132).
Patients with Medicare Part B and a secondary supplement plan could have out-of-pocket costs as low as $0 per month.
Patients with Medicare Part B and no secondary supplement plan will pay $15.58 per month once their yearly Medicare Fee-for-Service Part B deductible is met*
* Does not apply to patients enrolled in Medicare Advantage Plans.
