Selecting the Right ForeseeHome Candidates

ForeseeHome is appropriate for the types of patients you see every day, including those that have unilateral or bilateral intermediate dry AMD, or wet AMD in one eye and intermediate dry AMD in their fellow eye.

ForeseeHome is cleared for patients who have a best corrected visual acuity (BCVA) of 20/60 or better and who have the following intermediate dry AMD ICD-10 codes:

• H35.3112 (OD)
• H35.3122 (OS)
• H35.3132 (OU)

Patients who are taking AREDS2 vitamins and/or you have instructed to use the Amsler grid are often good candidates for ForeseeHome.
